Telehealth consults are offered only to patients who are physically in California or Arizona at the time of the visit. Arizona is telehealth only. There is no ONYX office in Arizona.

This page is a website privacy policy. It is not a substitute for the HIPAA Notice of Privacy Practices we provide for protected health information related to treatment. Ask the office for that notice. This is not legal advice.

What this site collects

We may collect information you give us on purpose:

  • Name, phone number, email address, and the message you send through our Contact page or by email
  • The state you will be in for a telehealth visit, if you tell us
  • Appointment or financing questions you send us

We may also collect limited technical information that the website creates when you visit:

  • Browser type, device type, and general location derived from your IP address
  • Pages viewed and how you reached the site
  • Cookies and similar tools used by our website host, Squarespace, to run the site, remember preferences, and understand basic traffic

Do not send Social Security numbers, payment-card numbers, or detailed medical histories through the public website or unencrypted email. Use the phone or the process our staff gives you for clinical information.

How we use it

We use this information to:

  • Answer questions and schedule in-person or telehealth visits
  • Confirm you can be seen in California or Arizona for a video visit
  • Operate, secure, and improve the website
  • Meet legal, professional, and record-keeping duties
  • Process a financing request only if you choose Cherry, which is offered for some in-clinic purchases and is not available for weight-loss medication or hormone replacement therapy

We do not sell your personal information.

Who we share it with

We share information only as needed to run the practice:

  • Squarespace, which hosts this website and related forms
  • Cherry, only if you apply for their payment plans (their own privacy policy applies to that application)
  • Instagram, Facebook, and X (@OnyxAW), if you choose to use those links or interact with our pages there
  • Service providers who help us with email, phones, fax, billing, or electronic health records, under a duty to protect the information
  • A pharmacy you choose, if we send a prescription after a visit
  • Professional advisers, insurers, or authorities when the law requires it or to protect patients, staff, or the public

We do not list a second clinic. We do not share your information to advertise an Arizona office. There is no ONYX office in Arizona.

Telehealth

If you ask for a video consult, we will ask you to confirm the state you are in for the entire visit. We cannot provide a clinical telehealth visit if you are outside California or Arizona when we connect.

Video visits may use a secure platform our staff names at booking. We do not invent or publish a third-party video brand on this site. A consult is an evaluation. It does not guarantee medication or a result.

Email and website forms are not for emergencies. Call 911 if you have a medical emergency.

Cookies and analytics

This site is built on Squarespace. Squarespace may set cookies that are needed for the site to work and cookies that measure visits. You can block cookies in your browser. Some site features may not work if you do.

We do not run a separate advertising pixel on this privacy page, and this policy does not authorize health-related ad tracking. If we later run ads, we will not use health information from a visit to target those ads without a lawful basis.

California privacy rights

If you are a California resident, you may ask us what personal information we have collected about you from the website, ask us to delete it where the law allows, and ask us not to sell or share it for cross-context advertising. We do not sell that information. We do not share it for cross-context behavioral advertising.

These website rights do not replace HIPAA rights for medical records. For records from a visit, ask the office for our Notice of Privacy Practices.

Health information

Information we collect as a health care provider to treat you, get paid, or run the practice is protected by HIPAA and similar state rules. That includes telehealth visits. We use and disclose that information as those laws allow, including for treatment, payment, and health care operations, and when you authorize it.

The public website is not a patient portal. Do not use it as your medical record.

How long we keep information

We keep website messages and technical logs only as long as needed to answer you, run the site, and meet legal or professional retention rules. Medical records are kept for the period required by California and Arizona law and our professional duties, which is usually longer than website logs.

Children

This site is for adults. We do not knowingly collect personal information from children under 13 through the website.

Security

We use reasonable administrative, technical, and physical safeguards, including SSL on this website. No method of transmission or storage is completely secure.

Changes

We may update this policy. The effective date at the top will change when we do. The current version will be posted at https://www.onyxaw.com/privacy-policy.
